Key Responsibilities
  • Manage a case load of patients to progress their hospital journey from admission to discharge.
  • Support the Discharge to Assess pathways, working internally and in partnership with the Local Authority to manage and progress a case load of patients who require ongoing assessments and treatment outside of an acute hospital setting.
  • Have knowledge of health and social care services, roles, and responsibilities, and apply this to support patient discharges.

The successful candidate will have a strong understanding of the Health and Social Care Act and its implications, as well as experience of working within a team and managing a group of patients.
