About the Role:

We are seeking an experienced Complex Healthcare Supervisor to join our care team in West Kent. As a key member of our team, you will work closely with our Care Manager to oversee our clients in the community, ensuring they receive high-quality care and support.

Key Responsibilities:

  • Collaborate with our Care Manager to setup care packages and introduce new staff to our clients
  • Maintain familiarity with the complex needs of each of our clients to ensure continuity and consistency as per their care plan
  • Supervise, shadow, manage competency assessments, conduct observations, and mentor our care assistants
  • Work with the care coordinator to maintain shift cover within the care package team
  • Perform clinical duties of personal care, medication administration, meal preparation, and light household duties
  • Participate in the on-call rota, shared with the wider team

Requirements:

  • Experience of supporting those with disabilities and complex health needs
  • Attend mandatory fully funded training and achieve a QCF level 3 Health and Social Care qualification within the first year of employment
  • Strong communication, excellent organisation, and problem-solving skills

About Continuity of Care Services:

Location: West Kent

Salary: £26,000 to £28,000 per annum, depending on experience

Hours: 40 hours per week, permanent contract
