Community Healthcare Leader

About the RoleWe are seeking a highly skilled and experienced Community Healthcare Leader to join our team at Kent Community Health NHS Foundation Trust. As a key member of our leadership team, you will be responsible for leading and managing a team of healthcare professionals in providing high-quality patient care within our community nursing services.
The successful candidate will have a proven track record of leadership and management experience within a similar field, with excellent communication and interpersonal skills. You will be able to motivate and inspire your team to deliver exceptional patient outcomes, while also developing and implementing effective policies and procedures to drive service improvement.
Key Responsibilities- Lead and manage a team of healthcare professionals in providing high-quality patient care within our community nursing services.
- Develop and implement effective policies and procedures to drive service improvement.
- Work closely with other stakeholders, including GPs, social services, and voluntary organizations, to ensure seamless delivery of care.
- Manage and lead change, ensuring that the needs of patients and staff are met.
- Monitor and evaluate the performance of the team, identifying areas for improvement and implementing changes as necessary.
- Registered healthcare professional (NMC registration) with a health-related degree and evidence of relevant postgraduate development at Level 6.
- Minimum of five years' experience in area of practice, with experience of leading and managing teams.
- Proven ability to develop effective partnerships to achieve practical outcomes across organisational boundaries.
- Excellent communication and interpersonal skills, with the ability to motivate and inspire others.
- Valid driving licence and access to a car for work purposes.
