Access Team Shift Manager

We are seeking an experienced and skilled Access Team Shift Manager to join our team at Navigo Health and Social Care CIC. As a key member of our integrated Access Team, you will play a pivotal role in improving the lives of individuals using our services, supporting them to access the most appropriate services for their needs.
Key Responsibilities- Facilitate the safe operation and management of the Adult Access team during allocated periods of responsibility.
- Ensure the practice of self and team reflects the assessment response times and other metrics and outcome measures.
- Design, deliver, and evaluate care plans.
- Attend and sometimes lead appropriate appointments and meetings, including MDTs, follow-up and assessment appointments, team meetings, etc.
- Undertake full mental health triage and assessment of care needs of patients presenting with complex clinical conditions.
- Determine management plans, and where appropriate, deliver short-term interventions for people who are referred with complex mental health needs based upon a comprehensive biopsychosocial, risk, and needs assessment of that individual.
- Determine and implement the appropriate treatment and risk management pathway for assessed individuals.
- Five years post-registration mental health work experience.
- Evidenced knowledge of relapse prevention.
- Three years supervision, management, and/or leadership experience in a health or social care environment.
- Experience using patient records system(s).
- Three years experience using clinical assessment tools.
- Three years experience formulating and delivering biopsychosocial interventions.
- Knowledge of the principles of non-medical prescribing and the use of patient group directives/protocols.
- Evidenced knowledge and understanding of trauma-informed, person-centred approaches.
