Social Care Specialist

This is an exciting opportunity to join Proud to Care Devon as a Social Care Specialist in the South West England region. As a Social Care Specialist, you will be responsible for providing high-quality social care services to individuals with complex care needs.
You will work closely with a multidisciplinary team to facilitate complex discharges from Derriford Hospital and review service users' care needs within the community. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
The successful candidate will have a competitive salary of £28,000 - £40,000 per annum, depending on experience, as well as access to continuous professional development opportunities. We also offer flexible working arrangements, including part-time hours, flexitime, and homeworking.
