Key Responsibilities- Provide near-patient medicine management services as part of our ward-based team, ensuring cost-effective and efficient services are maintained throughout patients' stays.
- Integrate into the ward multidisciplinary team, assisting in medication reconciliation, discharge planning, TTO medicine supply, and patient counseling.
- Assess patients' own drugs, conduct audits, order and dispense medications, and check supplies.
- Participate in extended hours, including weekend working, bank holidays, and late nights on a rotational basis.
