COPD Specialist Nurse or Physiotherapist

We are seeking an exceptional COPD Specialist Nurse or Physiotherapist to join our Inreach COPD Team at Worcestershire Acute Hospitals NHS Trust.
About the RoleThis is an exciting opportunity to work in a dynamic team, providing specialist care to patients with Chronic Obstructive Pulmonary Disease (COPD) in hospital and community settings.
Key Responsibilities- Act as a specialist, autonomous clinician for patients with COPD, providing comprehensive care and support.
- Provide patient monitoring, patient and carer education, and referral to other healthcare professionals as needed.
- Evaluate patient needs, planning, delivering, and evaluating all elements of COPD management.
- Contribute to respiratory clinics, both in acute and community settings.
- Construct specialist self-management plans for patients and assess and monitor clinical progress.
- Communicate effectively with patients, carers, and multidisciplinary teams to ensure coordinated care.
- Provide expert clinical education and support to healthcare professionals, patients, and carers.
- Contribute to the development of specialist protocols and care pathways for people requiring a respiratory service.
- Currently Registered Nurse with the relevant UK professional body (NMC).
- Meets the standards for continuing professional development as outlined by the relevant professional body.
- Evidence of relevant clinical specialist knowledge in respiratory care acquired through degree-level training or equivalent.
- Experience of working in a community setting, including respiratory medicine.
- Awareness of Pulmonary Rehabilitation Programme.
- Holds a current valid UK driving license and is able to travel within the post requirements.
- Able to work weekends on a rota basis and occasionally work outside of office hours.
