Job Title: Respiratory Specialist Nurse

We are seeking a highly skilled and experienced Respiratory Specialist Nurse to join our COPD Inreach Team. As a key member of our team, you will play a vital role in providing high-quality care to patients with chronic respiratory disease.

Key Responsibilities:
  • Provide comprehensive care to patients with COPD, including medication and inhaler technique reviews, self-management plans, and pulmonary rehabilitation.
  • Work closely with the COPD Virtual Ward and community COPD Team to ensure seamless care transitions.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with multidisciplinary teams to ensure effective communication and coordination of care.
  • Provide education and support to patients and their carers to empower them to manage their condition effectively.
Requirements:
  • Current registration with the NMC as a Registered Nurse.
  • Experience in respiratory care, preferably in a community setting.
  • Strong knowledge of COPD management and treatment options.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
