Job Summary

The Long Furlong Medical Centre is seeking a highly skilled and experienced Advanced Nurse Practitioner to join our team. As an autonomous practitioner, you will work independently and in conjunction with other healthcare professionals to assess, diagnose, and treat patients attending our primary care facility.

Main Responsibilities

Key responsibilities of this role include:

  • Providing high-quality patient care from initial history taking to diagnosis, treatment, and evaluation of care
  • Initiating direct referrals as necessary and working collaboratively with the multidisciplinary team to meet patient needs
  • Leading the nursing team and mentoring and supporting other nurses in developing and maintaining skills
  • Providing home visits to acute unwell patients and housebound CDM patients on a daily basis
  • Maintaining a safe and effective level of practice and bringing any deficits in training to the attention of the practice manager

Requirements

To be successful in this role, you will need:

  • Experience of working within a nursing team
  • Registered General Nurse qualification
  • Recognised NP qualification
  • Excellent communication and interpersonal skills
