Job Overview
The Isle of Wight Council's Adult Social Services department is looking for an experienced Community Care Coordinator to join our team. As a Community Care Coordinator, you will play a crucial role in supporting individuals to maintain their independence and well-being. You will work closely with a variety of stakeholders, including healthcare professionals, voluntary sector organizations, and community groups, to provide person-centered support and advocacy. About the Role
In this role, you will be responsible for assessing individuals' needs and developing personalized support plans that promote their independence, choice, and autonomy. You will also work collaboratively with other teams to ensure seamless service delivery and effective communication with stakeholders. Additionally, you will provide advice and guidance to carers and family members, as well as offering Carers Assessments in line with the Care Act 2014. To succeed in this role, you must have excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. A suitable candidate should be suitably qualified, hold up-to-date professional registration, demonstrate knowledge and understanding of The Care Act 2014, Mental Capacity Act 2005, Data Protection Act 1998, and evidence of ability to work according to practice guidance/procedure. A salary of approximately £25,000 per annum is offered, plus benefits including annual leave entitlement of 32 days p/a PLUS public holidays, Local Government pension scheme, and various other perks such as discounted Solent ferry travel and cycle to work scheme. An estimated salary range for this role is £22,500 - £28,000 per year.
