Requirements:
- Registered with the Health and Care Professions Council (HCPC)
- Proven experience in complex care management
- Exceptional communication and interpersonal skills
What We Offer:
A competitive salary of £45,000 - £55,000 per annum, depending on experience, and the opportunity to work with a reputable healthcare provider.
