Practitioner of Person-Centred Support

About This Role:
We are seeking a highly motivated Support Practitioner to join our team in delivering person-centred support services to individuals.
Key Responsibilities:
- Deliver person-centred support that meets the unique needs and preferences of each individual
- Develop and maintain strong relationships with individuals, families, and other stakeholders
- Collaborate with colleagues to ensure seamless delivery of support services
- Participate in ongoing training and development to enhance your skills and knowledge
- Contribute to the continuous improvement of our support services
Requirements:
To be successful in this role, you will need:
- A flexible and inspirational approach to delivering person-centred support
- Excellent communication and interpersonal skills
- Ability to work collaboratively as part of a team
- Strong problem-solving and analytical skills
- Basic knowledge of relevant legislation and policies
Salary and Benefits:
Competitive salary: £28,000 - £30,000 per annum
Plus a range of benefits, including:
- 35 days' holiday per year (pro rata)
- Time and a half payment for any time worked over 37.5 hours per week
- Carr Gomm will reimburse the SSSC fee for all new permanent employees applying for SSSC registration
- Recommend a Friend Scheme (T&Cs apply)
- Additional mileage contributions of 45p per mile
