At Digitaltidewave, we are seeking a Care Coordinator to join our team. The estimated salary for this role is £25,000 - £30,000 per annum.

This role involves providing delegated duties to patients in various settings, including routine wound care, venepuncture, and administration of insulin and eye drops following training.

The successful candidate will communicate effectively with District Nursing Team members, other health care professionals, and voluntary agencies discussing patient care as directed by the qualified nurse.

Key responsibilities include undertaking clerical work, maintaining a nursing bag with appropriate equipment, participating in health promotion programmes with patients and carers, and contributing to budget management.

We require candidates who possess excellent verbal and non-verbal communication skills, demonstrate empathy with patients, their families and carers, and ensure their views, choices and experiences influence thinking and individualised care provision.
