Hygea Homecare seeks an aspiring trainee Care Coordinator to join our growing team of compassionate care professionals. Our Care Coordinators are responsible for planning, coordinating and delivering an array of home care services for our clients.

**Responsibilities**:

- Develop and implement individualised care plans that meet the individual needs of our clients.
- Communicate and collaborate with healthcare professionals, family members and caregivers.
- Facilitate care provision, including activities, errands, meals and other required services.
- Coordinate staff to ensure the seamless transition of client care.
- Keep up-to-date records on client files.
- Evaluate the effectiveness of care plans.
- Facilitate health care team meetings and coordinate care with other professionals.
- Act as an advocate for clients.
- Educate family members on the available services and resources
- Provide cover on the ground where required to ensure smooth service delivery.
- Identify training requirements and report to relevant management.

**Requirements**:

- A Minimum Level 3 in Health and Social Care and willing to work towards Level 4 Lead Practitioner Qualification.
- At least one year of senior care experience
- Professional, empathetic and strong desire to help others.
- Ability to work independently and manage caseloads.
- Excellent interpersonal, communication and organisational skills.
- Must possess strong problem-solving skills and flexibility.
- A UK driver's Licence and access to a vehicle.

**Job Benefits**:

- Remuneration: £22,425 p/a
- Permanent contract.
- 28 Days Holiday per year.
- Mileage payments at 40p per mile between client visits.

**Locations we operate in**:
You will be primarily based at our office in Long Eaton, and you will be supporting clients and staff in the following locations: Long Eaton, Sandiacre, Sawley, Risley, Breaston, and Draycott in Derbyshire.

**Working Hours**:
You will be scheduled to work 40 hours per week, including breaks, a mixture of earlies, late and every other weekend on rotation, to ensure smooth care delivery.
