Cardiology Outreach Advanced Nurse Practitioner

To be the Neurosciences Co-ordinator and to ensure that the Trust's targets are adhered to and executed to the highest standards through liaison with clinicians, healthcare professionals, managers, administrative and secretarial staff, both within and outside of the organisation, as well as patients and relatives.
Main Duties- Ensure effective use of the beds within the four Neurosciences wards and the day unit.
- Ensure that emergency, elective and tertiary referrals are given equal consideration, working with clinicians and the NOTSS Clinical Coordinator to decide on bed allocation.
- Provide support to the wards by liaising with the Discharge Coordinator to ensure that patients vacate their beds in a timely manner on the day of discharge, and with the ward shift coordinators, to ensure that patients are admitted promptly to the vacated beds.
- Work in collaboration with clinical staff to appropriately transfer patients from NICU/AICU.
- Work closely with the Patient Access Lead / Service Support Manager to ensure that all elective patients are admitted in a timely manner, within local and national access targets and that the most appropriate use of all available resources is made.
