Youth Support Coordinator

**About the Role**
As a Youth Support Coordinator at Service Care Solutions, you will have the opportunity to make a real difference in the lives of young people preparing to leave care.
You will work closely with care leavers, providing support and guidance to help them develop essential life skills for independence.
Responsibilities:
- Establish relationships with care leavers and ensure their views are heard and needs are central to all support efforts.
- Participate in the assessment, preparation, and review of pathway plans to equip young people with essential life skills.
- Facilitate access to relevant services, working collaboratively with partner agencies to support care leavers.
- Maintain comprehensive records of interactions and services provided.
Requirements:
- NVQ Level 3 in Advice and Guidance, Health and Social Care, or equivalent education.
- Strong understanding of young people's issues, assessment methods, and education/employment trends.
- Excellent communication skills, flexibility, adaptability, and commitment to equal opportunities.
Salary: £27,000 - £32,000 per annum depending on experience.
