Social 1st is seeking a highly skilled Youth Transition Coordinator to join our Leaving Care Team. This is an exciting opportunity for an individual who is passionate about supporting young people in transition.

The ideal candidate will have excellent assessment and support skills, with the ability to provide guidance and advice to promote the potential of care leavers.

We estimate that this role will attract a salary of £17.17 per hour, which reflects the skill level and experience required.

The successful candidate will work closely with our team to enhance the capacity of care leavers to realise their full potential and become successful in all aspects of their lives.

Key Responsibilities:

  • Enhance the capacity for care leavers to realise their full potential and become successful in all aspects of their lives.
  • Provide a targeted, outcome-focussed service to young people who are legally defined as care leavers.
  • Provide qualitative assessments, ongoing support, advice and guidance to promote the potential of care leavers to develop the skills necessary to live independently.
