Salary: For this role, we offer a competitive hourly rate of £30-£38, with additional out-of-hours enhancements.
Job Description:
As a Bank Health Visitor, you will join our team in central Lancashire, working flexible hours to meet the needs of our clients. Your responsibilities will include delivering the Healthy Child Programme, assessing health needs, and developing individualized care plans.
Key Responsibilities:
- Deliver the Healthy Child Programme, including assessment of health needs and implementation of care plans
- Use the Family Health Needs Assessment tool to plan and evidence care
- Devising individualized care plans for each child, involving parents and caregivers in the planning process
