About the Role

We are seeking a skilled and experienced Mental Health Practitioner to join our Calderdale Enhanced Team, a Community Mental Health Team operating in Calderdale. As a key member of our team, you will play a crucial role in delivering high-quality, recovery-focused care to individuals accessing our service.

The successful candidate will have a strong background in assessing and managing complex mental health problems, with experience of working holistically with older adults in a bio-psycho-social model of care. You will also have experience of working in an Assertive Community Treatment (ACT) model and be able to demonstrate skills in assessing and managing risk in a variety of settings.

We are looking for someone who is passionate about delivering person-centered care and is able to work collaboratively with our multidisciplinary team to provide comprehensive support to service users. You will have excellent communication and interpersonal skills, with the ability to build strong relationships with service users, carers, and other professionals.

The role will involve undertaking initial and risk assessments, formulating and reviewing care plans, and implementing a planned programme of care to ensure a high standard is achieved. You will also be responsible for advising and supporting service users and carers to promote health and well-being and to prevent illness.

Key Responsibilities

Requirements
  • RMN or Social Worker/Occupational Therapist with current NMC or HCPC registration.
  • Experience of working in a Community Mental Health Team, with a strong background in assessing and managing complex mental health problems.
  • Experience of working holistically with older adults in a bio-psycho-social model of care.
  • Experience of working in an Assertive Community Treatment (ACT) model.
  • Ability to demonstrate skills in assessing and managing risk in a variety of settings.
