About the Role

We are seeking a skilled and motivated Physiotherapy Assistant Practitioner to join our Acute Medical Rehabilitation team at Hywel Dda University Health Board. As a key member of our team, you will play a vital role in supporting the recovery and rehabilitation of patients across our medical wards.

Working closely with our multi-disciplinary team, including Physiotherapists, Occupational Therapists, Social Workers, Discharge Liaison Nurses, Nurses, and Doctors, you will be responsible for supporting and completing holistic patient assessments, with consideration of rehabilitation needs and strategies/options to aid a safe and timely discharge.

You will also be involved in facilitating continuity of patient care, including a degree of out-reaching into the community, and will play a pivotal role in delivering rehabilitation to patients admitted to the medical wards to promote each patient's independence.

Key Responsibilities

  • Support and complete holistic patient assessments, considering rehabilitation needs and strategies/options for safe and timely discharge.
  • Facilitate continuity of patient care, including out-reaching into the community.
  • Deliver rehabilitation to patients admitted to the medical wards to promote independence.
  • Work as part of a multi-disciplinary team to support patient care.
