Mental Health Practitioner

Hywel Dda University Health Board is seeking a full-time Community Mental Health Practitioner with a qualification and registration as a RMN nurse, Occupational Therapist, or Social Worker.
The selected candidate will join the Integrated Community Mental Health Team, based at Wellfield Resource Centre, in Carmarthen. The position is permanent and will be full-time, with 37.5 hour shifts over a 9am to 5pm shift pattern rota, Monday to Friday.
The team operates 5 days a week, working through an Assessment and Recovery approach with people accessing Secondary Mental Health Services.
Main Duties- Assessing care needs and developing, implementing, and evaluating programmes of care for a defined clinical caseload, including care treatment planning under the Mental Health Wales Measure 2010.
- Working without direct supervision for their delegated caseload and developing positive working relationships between healthcare professionals and statutory and voluntary agencies.
As a Community Mental Health Practitioner, you will be responsible for providing high-quality care to individuals with mental health needs in the community.
Requirements- Professional qualification and registration as a RMN nurse, Occupational Therapist, or Social Worker.
- Post-basic education in the area of practice to postgraduate diploma level.
- Post-registration experience.
