About the Role

We are seeking a highly motivated and dedicated Comprehensive Care Coordinator for Indigenous Population to join our team. As a key member of our HIV clinical case management team, you will work collaboratively to prevent HIV and promote overall health for your client base.

Your key responsibilities will include:

  • Prioritizing client care plans and securing housing and income security
  • Updating the Sexual Health/BBI team on client engagement and disengagement
  • Assisting with organizing appointments, travel, and related activities to maintain client engagement in HIV care

You will also support clients with access to peer-based programming and initiatives, linkage to primary care, risk reduction support strategies, and referrals to promote safer behaviors. Additionally, you will champion and lead aspects of our HIV Anti-Sigma Working group and support the development of peer support groups for individuals engaging in high-risk behaviors.

We offer a competitive salary of $27.00 per hour, as well as benefits including dental care, extended health care, on-site parking, paid time off, and a wellness program. This is a full-time position with a schedule from Monday to Friday. Ability to commute/relocate to Surrey, BC V3R 1V9 is required.
