About the Role

We are seeking an experienced Peer Worker/Coach to join our Community Services team at Camden and Islington NHS Foundation Trust. The successful candidate will be part of a dynamic team working with organizations throughout the community to bring a population health approach into action.

Job Responsibilities
  • To provide, create, and deliver a range of peer support activities and care planning for service users who meet the MH Population Team criteria.
  • To review and manage risk as part of the support provided, seeking support from the MDT where needed.
  • To be involved in developing tools and procedures for new and existing Peer Coaches as well as updating existing materials to reflect changes to Trust procedures and policies.
Main Duties
  • Permanent full/part-time Peer Working/Coaching opportunities.
  • Work with service users to identify their needs and develop personalized plans to achieve their recovery goals.
  • Collaborate with healthcare professionals to ensure seamless delivery of services.
Person Specification
  • Good general education with Maths and English to GCSE standard or equivalent.
  • Good written and spoken English.
  • Completion of C&I Peer Coach training programme or equivalent training/experience.
  • Experience of individual peer work/coaching and awareness of local services and supports.
Salary and Benefits

The salary for this role is £31,944 - £34,937 per annum, inclusive of HCAS. As a member of our team, you will also have access to a comprehensive benefits package, including pension scheme, annual leave, and career development opportunities.
