We are looking for a skilled Healthcare Professional for Community Care to join our vibrant team at the Northern Care Alliance NHS Foundation Trust. As a key member of our community nursing team, you will play a vital role in enhancing healthcare delivery, providing exceptional care to patients in their own environments.

This is a fantastic opportunity to develop your clinical expertise and leadership skills, working closely with junior staff and contributing to the growth and collaboration of our team. You will have the autonomy to assess and attend to the diverse health needs of our community, including patients facing end-of-life challenges and those with long-term or complex conditions.

The estimated salary for this position is £37,500 - £50,000 per annum, depending on experience. We offer a comprehensive benefits package, including ongoing training and development opportunities, to support your professional growth and well-being.

To be successful in this role, you will need to demonstrate excellent communication skills, both written and verbal, as well as the ability to plan and organise own work effectively. You will also require a strong understanding of community working and care pathways, with excellent interpersonal skills and leadership potential.
