About This Role

This is an exciting opportunity to join Advantage Healthcare as a Community Care Coordinator Lead. In this role, you will play a key part in shaping the direction of our community care services and ensuring that our clients receive high-quality care.

Key Responsibilities

  • Leading a team of care workers to deliver high-quality care to clients
  • Developing and implementing effective care plans in partnership with clients, families, and other healthcare professionals
  • Monitoring and evaluating the effectiveness of care plans and making recommendations for improvement
  • Maintaining accurate records and reports as required
  • Contributing to the development and implementation of policies and procedures that support the delivery of high-quality care

Requirements

To be successful in this role, you will need:

  • A degree in a relevant field (e.g., nursing, social work, or healthcare management)
  • Significant experience in a leadership or management role within the healthcare sector
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Strong organizational and time-management skills

Salary and Benefits

The salary for this role is £40,000 - £55,000 per annum, depending on experience. We also offer a range of benefits, including:

  • 22 days annual leave (increasing to 25 days after 3 years' service)
  • Occupational maternity pay and adoption pay
