We are looking for a skilled and empathetic Support Worker to join our Specialist Care Team in Winchester. As a key member of our team, you will play a vital role in supporting an individual to achieve their life goals and aspirations.

Key Responsibilities:
- Deliver person-centred care that promotes independence and well-being
- Assist the individual with daily living tasks, such as personal care and domestic duties
- Support the individual to access community activities and social events
- Collaborate with other professionals to develop and implement personalized care plans
Requirements:
- Relevant experience in care work or a related field
- Valid full driving licence (automatic)
- Enhanced DBS Disclosure
- Able to work independently and as part of a team
