Job Description:

We are seeking an experienced Social Worker to join our Adults Integrated Urgent Care Team. As a key member of the team, you will be responsible for supporting adults to live well at home, managing a caseload, conducting assessments and support plans, and signposting to internal and external services as necessary.

Required Skills and Qualifications:

">

To be successful in this role, you will need a degree or equivalent in social work, current registration with Social Work England, and significant frontline experience in social services within the UK. Excellent communication skills and the ability to build effective relationships with other disciplines are essential.
