At Entrust Social Care Ltd, we are seeking a dedicated Adult Care Professional to join our team. This is a fantastic opportunity for an experienced Social Worker to work in a dynamic environment, providing high-quality care to vulnerable adults.

Salary: £25-£35 per hour (dependent on experience)

About the Role

We are looking for a qualified Social Worker with a minimum of 2 years+ UK Social Work experience within an Adult Social Work team. As a key member of our team, you will be responsible for overseeing hospital discharges under the Discharge to Assess Framework / Care Act.

You will work as part of a multi-disciplinary team to provide support to older people with diagnosed memory problems who need social care because of their mental health and/or memory problems.

  • Collaborate with colleagues from health and social care to respond to urgent requests for assessment/monitoring/support to people with a diagnosis of dementia or significant mental health disorder who are presenting with a physical health problem to prevent hospital admission and to facilitate a co-ordinated discharge back into the community.
  • Work in collaboration with the intermediate care team to facilitate reablement when identified.

Key Responsibilities:

  • Ensure that the social care needs of people are met within the eligibility criteria of the department, by working in partnership with people who use the service, their carers, professionals, and other agencies, and service providers.
  • Support in delivering and maintaining high standards of performance ensuring continuous development and supporting people to fulfill their roles to maximum effectiveness within allocated resources.

The successful candidate will need to hold a DipSW or equivalent, must be registered with the HCPC, have a current DBS, and be a car driver.
