Counselor and Care Coordinator

Remedy Social Care is seeking a dedicated and experienced Social Worker to fill a locum position within the Hospital Discharge and Assessment Team in Gloucestershire.
This role involves working closely with healthcare professionals to ensure smooth patient transitions from hospital to community settings. The successful candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
Responsibilities- Coordinate casework, ensuring timely interventions and effective outcomes.
- Develop and maintain relationships with stakeholders, including healthcare professionals and community partners.
- Ensure compliance with relevant policies and procedures.
The ideal candidate will have experience working with vulnerable adults and possess a strong understanding of safeguarding principles. A consistent pay rate of £35 per hour applies, with expenses for hard-to-recruit geographical areas – including £150 accommodation/commuter mileage allowance.

The contract is for at least 3 months with two weekly reviews of outstanding vacancies. Remedy Social Care is an APSCo Standard agency and is committed to safeguarding and promoting the welfare of the vulnerable. Successful applicants will be required to undertake an Enhanced Disclosure via the DBS.
Requirements- Qualified Social Worker with experience working with vulnerable adults.

Estimated Salary: £13,400 - £16,600 per month (based on 5 days/week, 30 hours/week).
