Job Title: Safeguarding Specialist Nurse for Adults

We are seeking a highly skilled and experienced Safeguarding Specialist Nurse for Adults to join our team. As a key member of our Complex Discharge Team, you will play a vital role in ensuring the safe and effective discharge of patients from hospital.

About the Role

This is an exciting opportunity for a registered nurse with significant experience in an acute hospital care setting to join our team. You will have extensive knowledge of the discharge process and be confident in completing holistic, nursing needs-based assessments.

You will work closely with our multidisciplinary team, patients, and their families to ensure the timely completion of complex discharge assessments and service provision. Your expertise in safeguarding and discharge to assess will be invaluable in ensuring the best possible outcomes for our patients.

Key Responsibilities
  • Support the band 7 and band 6 Complex Discharge Nurses in ensuring the timely completion of complex discharge assessments and service provision.
  • Have an awareness of complex discharge, including continuing healthcare processes, end-of-life care, mental capacity, DOLS, and safeguarding.
  • Be confident in completing holistic, nursing needs-based assessments and liaising with all MDT, patients, and their families.
Person Specification

We are looking for a nurse with:

3 years post-registration experience in an acute or community ward.Excellent communication and interpersonal skills.Ability to work autonomously and as part of a team.Computer literacy and knowledge of STRATA-RIO-Nerve Centre (desirable).NMC registration.What We Offer
