Job Title: Safeguarding Specialist Nurse for Adults

We are seeking a highly skilled and experienced Safeguarding Specialist Nurse for Adults to join our Complex Discharge Team. As a key member of our team, you will play a vital role in ensuring the safe and effective discharge of patients from hospital.

About the Role

This is an exciting opportunity for a dynamic and innovative nurse to join our team and contribute to the delivery of high-quality patient care. As a Safeguarding Specialist Nurse for Adults, you will be responsible for:

  • Completing holistic, nursing needs-based assessments to ensure the safe and effective discharge of patients
  • Liaising with multidisciplinary teams, patients, and their families to ensure seamless care transitions
  • Developing and implementing individualized discharge plans to meet the unique needs of each patient
  • Collaborating with other healthcare professionals to ensure the timely completion of complex discharge assessments and service provision
Requirements

To be successful in this role, you will need:

  • 3 years post-registration experience in an acute or community ward
  • Significant experience in discharge planning and safeguarding
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
