We are seeking a highly skilled and experienced Band 6 nurse to join our Complex Discharge Team. As a registered nurse, you will have significant experience in an acute hospital care setting and a strong understanding of the discharge process.

The successful candidate will be responsible for ensuring the safe, effective, and timely discharge of patients from hospital. This will involve working closely with the multidisciplinary team, patients, and their families to complete holistic nursing needs assessments and develop discharge plans.

Main duties of the job

  • Support the band 7 and band 6 Complex Discharge Nurses to ensure the timely completion of complex discharge assessments and service provision
  • Have an awareness of complex discharge, including continuing health care processes, end of life care, mental capacity, DOLS, and safeguarding
  • Be confident in completing holistic nursing needs assessments and developing discharge plans
  • Liaise with the multidisciplinary team, patients, and their families to ensure the safe and effective discharge of patients
