The Role: Social Care Worker
We are seeking a skilled and compassionate Adult Care Specialist to join our team in Bexleyheath, UK.
Job Overview:
As an Adult Care Specialist, you will work with older people, adults with disabilities, and their carers to assess their needs and develop tailored care plans. You will be part of a multi-disciplinary team that prioritizes client and carer wishes in decision-making and care planning.
Main Responsibilities:
- Care Needs Assessment: Conduct thorough assessments of the social care/rehabilitation needs of older people, including those with mental health needs and physical disabilities, and identify suitable solutions within a preventative framework.
- Caseload Management: Manage a caseload as allocated by the Operational Manager/Supervisor and collaborate with colleagues to deliver high-quality social care services.
- Care Planning: Develop and implement individual care plans in conjunction with clients, their carers, and professional colleagues, providing ongoing support and advice as needed.
About You:
- A level 2 or equivalent qualification in Care or related subject is desirable.
- Demonstrable relevant experience working with older people, adults with disabilities, and carers.
- Experience dealing with members of the public in a Health or Social Care context.
Salary and Benefits:
