Job Description

Corus Consultancy is seeking a highly skilled and compassionate Community Support Specialist to join our team in Bexleyheath. As a valued member of our team, you will be responsible for providing exceptional care and support to older people, including those with mental health needs, and individuals with physical disabilities, within their own homes or community settings.

Key Responsibilities
  1. Provide personalized support and care to service users, tailoring your approach to meet their unique needs and preferences.
  2. Work collaboratively with multi-disciplinary teams to deliver timely and effective care plans that maximize independence and promote a 'own bed is best' ethos.
  3. Develop and implement individual care plans in consultation with clients, carers, and professional colleagues, providing ongoing support and guidance as needed.
  4. Conduct regular reviews and assessments to identify areas for improvement, ensuring continuous quality enhancement.
  5. Build strong relationships with service users, carers, and professional colleagues, fostering trust and open communication.
Requirements
  • Demonstrable experience working with older adults, individuals with disabilities, and carers, preferably in a health or social care setting.
  • In-depth understanding of the needs of older people, individuals with disabilities, and/or those with mental health issues.
  • Excellent communication and interpersonal skills, with the ability to build rapport with service users and colleagues alike.
  • Strong analytical and problem-solving skills, with the ability to think creatively and develop innovative solutions.
Salary and Benefits

The salary for this role is £29,000 - £33,000 per annum, commensurate with experience. We offer a comprehensive benefits package, including opportunities for career development and growth within our organization.
