Job Summary

We are seeking a highly skilled and motivated Community Staff Nurse to join our District Nursing Team in Trafford. As a Community Staff Nurse, you will play a vital role in providing high-quality nursing care to adults in their own homes and in various community settings.

About the Role

As a Community Staff Nurse, you will work closely with the Multi-Disciplinary Team (MDT) to ensure effective coordination of patient care, demonstrating a positive impact on our patients. You will be part of the Integrated Community Team, working for Manchester University NHS Foundation Trust, a leading integrated health, teaching, research, and public sector partnership organization in the Manchester region.

Key Responsibilities
  • Provide high-quality nursing care to adults in their own homes and in various community settings.
  • Work closely with the MDT to ensure effective coordination of patient care.
  • Participate in the development of care plans and contribute to the delivery of care.
  • Collaborate with other healthcare professionals to ensure seamless care transitions.
  • Contribute to the development of the team and the organization through participation in quality improvement initiatives and professional development activities.
Requirements
  • RGN qualification and registration with the NMC.
  • Recent community experience and recent CPD.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility and willingness to work in a fast-paced environment.
