Job Summary:

We are seeking a skilled Mental Health Practitioner to join our Children and Young People's (CYP) Emergency and Urgent care provision. As part of the NHS 5 Year Forward View, Cheshire and Wirral Partnership has developed the crisis response services for CYP.

Key Responsibilities:

  • Conduct comprehensive mental health risk assessments for CYP in crisis
  • Develop and implement crisis interventions to promote safety and support young people and their family/carers
  • Liaise with other services and external agencies to ensure effective pathways of care
  • Work collaboratively to promote safety and support young people through face-to-face work and via the CWP Trust Wide Crisis Line

Requirements:

  • Professional registration with HCPC, Social Work England or NMC
  • Experience of assessing and managing risk within a mental health or social care setting
  • Ability to work collaboratively as part of a team
  • Excellent communication and interpersonal skills
