As a key member of our healthcare team, you will play a vital role in delivering high-quality care services to patients across three sites: Brooklands, Whitehouse, and Neath Hill surgeries.

About the Role

This is an exciting opportunity for a qualified nurse with experience in managing long-term health conditions, including Diabetes, COPD, and Asthma. You will work collaboratively with our team to deliver exceptional nursing care and contribute to maximizing Practice income through QOF and enhanced service contracts.

Key Responsibilities
  • Conduct venepuncture, blood pressure checks, ECGs, height/weight/BMI measurements, and other essential clinical tasks.
  • Deliver health promotion and education programs to patients, focusing on prevention and early management of long-term health conditions.
  • Administer vaccinations, including childhood and adult immunizations, as well as provide wound care and contraception services.
  • Participate in cervical screening programs and manage long-term condition patient records accurately and efficiently.
Requirements
  • Bachelor's degree in Nursing or equivalent qualification.
  • Minimum 1 year of experience in managing long-term health conditions.
  • Familiarity with Patient Group Directions (PGDs) and associated policies.
  • Excellent communication and record-keeping skills.

Estimated salary range: £30,000 - £45,000 per annum, depending on experience.
