Job Title: Clinical Care Specialist

We are seeking a highly skilled and dedicated Clinical Care Specialist to join our team at the Central & North West London NHS Foundation Trust. This is an exciting opportunity for a qualified Registered Nurse (Adult) to work in a dynamic and fast-paced environment, providing high-quality clinical care to patients.

The ideal candidate will have a real passion for delivering excellent patient care and be self-motivated, flexible, and able to work well within a team but also autonomously. You will not need to have previous prison experience as we will provide you with a full induction and training.

You will work within the Primary Care Team, which is varied and includes triaging, emergency care and treatment, health promotion, end of life/palliative care, effective care planning, and liaising with the multidisciplinary team. Every day will bring new challenges and new experiences, but you'll have all the support you need to deliver the best care possible from both the healthcare team and the prison team.

Main Duties:

  • Be part of a multi-disciplinary team delivering holistic health care to the prison population based on a primary care model.
  • Assess, plan, implement and evaluate high-quality evidence-based care within the boundaries of role.
  • Monitor long-term conditions, deliver health promotion and screening, and respond to medical emergencies.

Salary: £35,000 - £45,000 per annum
