About the Role

We are seeking an experienced Neonatal Community Outreach Nurse to join our team. As a key member of our multidisciplinary team, you will play a vital role in delivering safe and effective care to our babies and their families within their own home in the community.

Our service offers home phototherapy, home oxygen, home nasogastric feeding, and care for preterm infants, as well as other deliveries of care. You will work collaboratively with our neonatal and transitional care teams to support early discharge and then carry out home visits regularly throughout the working week.

As a Neonatal Community Outreach Nurse, you will support families with prescription changes or drugs used in the community, dependent on blood test results, and organize TTO's for drugs difficult to access for community pharmacies. You will also advise on changes in nutritional needs of the baby and weaning, and empower and educate families to learn new skills and become competent in caring for their baby at home.

Key Responsibilities
  • Deliver safe and effective care to babies and their families within their own home in the community
  • Work collaboratively with our neonatal and transitional care teams to support early discharge
  • Carry out home visits regularly throughout the working week
  • Support families with prescription changes or drugs used in the community
  • Advise on changes in nutritional needs of the baby and weaning
  • Empower and educate families to learn new skills and become competent in caring for their baby at home
Requirements
  • Neonatal experience with QIS or equivalent
  • Experience looking after families in the community
  • Teaching/education for staff and families
