About the Role

We are seeking an experienced Community Health Nurse to join our team at Sandwell and West Birmingham NHS Trust. As a Community Health Nurse, you will play a vital role in delivering high-quality care to families in their own homes.

Main Responsibilities
  • Support the facilitation of early discharge of well preterm babies and the home nursing of babies requiring specialist nursing intervention.
  • Carry out blood sampling in the patient's home, thus preventing unnecessary return visits to the hospital.
  • Support families with prescription changes or drugs used in the community, dependent on blood test results and organizing TTO's for drugs difficult to access for community pharmacies.
  • Offer advice to parents on non-prescription/over-the-counter medicines.
  • Advise on changes in nutritional needs of the baby and weaning.
  • Work in conjunction with other members of the multidisciplinary team and initiate transfer of care to other services.

We run a 7-day service 08:00-17:00, excluding Bank Holidays, where we work in partnership with our neonatal unit to empower and educate our families to take their baby home sooner, reducing the risk of hospital-acquired infection, separation, and outgoings for the family.

We work collaboratively with our neonatal and transitional care teams to support early discharge and then carry out home visits regularly throughout our working week and support our colleagues on the neonatal unit as and when required.
