Community Health Nurse

We are seeking a highly motivated and skilled Community Nurse to join our North Unplanned Community Nursing Team.
This is an exciting opportunity to work in a dynamic and innovative environment, providing nursing care to patients in their own homes.
The successful candidate will be responsible for responding quickly to patients with a variety of health needs, providing nursing care to achieve the best outcomes for patients, and working collaboratively with a multidisciplinary team to achieve patient independence and well-being.
Key Responsibilities:- Providing high-quality nursing care to patients in their own homes.
- Responding quickly to patients with urgent health needs.
- Working collaboratively with the multidisciplinary team to achieve patient outcomes.
Requirements:
The successful candidate will require a full driving license and access to a car, as well as excellent communication and interpersonal skills.
