Community Public Health Nurse

We are seeking a highly skilled and enthusiastic Community Public Health Nurse to join our team at Birmingham Community Healthcare NHS Foundation Trust. As a key member of our Health Visiting and Early Years team, you will play a vital role in delivering high-quality services to children and families in Birmingham.
Main Responsibilities- Conduct health needs assessments and screenings for children, providing prompt feedback and escalating concerns as necessary.
- Participate in the planning, delivery, and evaluation of health promotion activities, utilizing evidence-based practice to support individuals, families, groups, and communities.
- Contribute to the Early Help offer, supporting families to access additional support as required, in line with the Early Help framework.
- Contribute to the safeguarding of children at risk, adhering to appropriate Safeguarding policies and procedures.
- Participate in and promote childhood immunization programs as appropriate.
- Engage in the Trust's staff nurse development program and other relevant learning opportunities.
- Work within the NMC code of professional conduct.
- Support Health Visiting Teams in managing children with additional medical needs, completing Educational and Health Care plans, and preparing families for transition to education.
- Deliver evidence-based packages of care for children, in line with the BFS Standard Operational Procedures for Health Visiting and clinical pathways.
- Liaise with other Multi-Disciplinary Teams to ensure effective communication and coordination of care.
- Support the Health Visiting and Early Years teams in delivering Early Help interventions and completing Early Help Plans as required.
- Maintain accurate and contemporaneous electronic patient records for children and families, adhering to NMC requirements and Trust Record Keeping policies and guidelines.
- Communicate complex information to other professionals and clients, utilizing well-developed oral communication skills, including negotiation, empathy, and reassurance.
- Undertake clinical audits as directed by Health Visitor/Health Visitor Team Leader, supporting action plans according to findings.
- Recognize and respond appropriately to identified or potential concerns regarding a child's developmental or health needs.
- Work with and support families through difficult situations, such as illness and bereavement.
- Plan, organize, and run clinics to support the health and developmental needs of children as required.
- Ensure data is inputted onto a data collection system accurately and on time, as required by the Trust.
- Deliver care in various settings, including children's centers, clinics, GP surgeries, and clients' homes, requiring lone working in the community setting.
- Maintain effective working relationships and communicate with the BFS team and other professionals.
- Continuously update professional knowledge and skills, attending mandatory training and relevant updates as required by the Trust.
- Cooperate and assist in research and survey programs related to the client group.
- Participate in the teaching, training, and assessment of support staff within the cluster and in the clinical placement of all learners pre-registration.

- First-Level Registered Nurse
- Evidence of relevant professional development post-nurse registration
