Job Title: Band 5 Community Nurse

We are seeking a highly skilled and dedicated Band 5 Community Nurse to join our team at Birmingham Community Healthcare. As a Community Nurse, you will play a vital role in delivering high-quality healthcare services to children and families in our community.

Job Summary

The successful candidate will be responsible for undertaking health needs assessments and screening for children, participating in the planning, delivery, and evaluation of health promotion activities, and contributing to the safeguarding of children at risk. You will also work closely with our Health Visiting Teams to support families and provide evidence-based packages of care.

Key Responsibilities
  • Undertake health needs assessments and screening for children as directed by the named or delegating Health Visitor.
  • Participate in the planning, delivery, and evaluation of health promotion activities for individuals, families, groups, and communities.
  • Contribute to the safeguarding of children at risk in line with appropriate Safeguarding policies and procedures.
  • Work closely with Health Visiting Teams to support families and provide evidence-based packages of care.
  • Participate in the Trust staff nurse development program and other related Trust-based learning opportunities.
Requirements
  • First Level Registered Nurse qualification.
  • Evidence of relevant professional development post-nurse registration.
  • RSCN/Child Branch or Learning Disabilities qualification.
Skills and Experience
  • Ability to demonstrate sound knowledge and application of up to date evidence-based clinical trends and practice.
  • Experience and ability to demonstrate an understanding of working with children and families in a clinical and/or community setting.
  • Evidence of recent professional development.
