Healthcare Practitioner

We are seeking a skilled and compassionate Healthcare Practitioner to join our team at Practice Plus Group. As a key member of our healthcare team, you will be responsible for delivering high-quality patient care and providing support to our patients and their families.
Key Responsibilities- Assess and prioritize patient needs, providing individualized care and support
- Work collaboratively with other healthcare professionals to ensure seamless patient care
- Develop and implement effective care plans, taking into account patient preferences and needs
- Monitor patient progress, making adjustments to care plans as necessary
- Communicate effectively with patients, families, and healthcare teams to ensure optimal care
- Registered Nurse or Paramedic with current registration with the NMC or HCPC
- 6 months post-qualification experience, including experience in an acute setting
- Evidence of Continuous Professional Development (CPD)
