About the Role
We are seeking a highly motivated and passionate Band 5 Community Nurse to join our dynamic District Nursing Service. As a key member of our team, you will provide clinically effective care to patients in their own homes and communities.
Main Responsibilities
- Deliver high-quality evidence-based care to patients with complex needs.
- Work collaboratively with other healthcare professionals to achieve seamless patient-centered care and treatment.
- Travel between patients' homes in towns and rural areas to provide timely and effective care.
