Job DescriptionWe are seeking a dedicated and compassionate Community Healthcare Assistant to join our team. As a key member of our patient care coordination team, you will be responsible for ensuring seamless care delivery and promoting optimal patient outcomes. Your excellent communication skills, attention to detail, and ability to work independently will make you an asset to our team.
Salary: £24,500 - £26,500 per annum (dependent on experience)
Key Responsibilities- Coordinate patient care activities and ensure timely delivery of services
- Communicate effectively with patients, families, and healthcare professionals to promote optimal care outcomes
- Maintain accurate and detailed records of patient interactions and care delivered
- Work collaboratively with our multi-disciplinary team to ensure seamless care delivery
Requirements
- GCSE English and Maths or equivalent qualification
- Minimum NVQ/Diploma Level 2 in Healthcare or equivalent
- Experience in delivering care in a healthcare or people-centered environment
- Ability to work independently and as part of a team
