About the Role

We are seeking a highly skilled and compassionate Clinical Support Worker to join our team at South Warwickshire University NHS Foundation Trust. As a key member of our community healthcare team, you will play a vital role in delivering high-quality patient care and supporting our registered professionals in their duties.

Job Summary

This is an exciting opportunity for a motivated and empathetic individual to work in a dynamic and supportive environment. You will be responsible for providing comprehensive care to patients in their own homes, under the direction of our registered professionals. Your excellent communication skills, attention to detail, and ability to work independently will make you an asset to our team.

Salary: £23,000 - £25,000 per annum (dependent on experience)

Key Responsibilities
  • Provide personalized care to patients in their own homes, adhering to our trust's standards and policies
  • Work collaboratively with our multi-disciplinary team to ensure seamless care delivery
  • Communicate effectively with patients, families, and healthcare professionals to promote optimal care outcomes
  • Maintain accurate and detailed records of patient interactions and care delivered

Requirements

  • GCSE English and Maths or equivalent qualification
  • Minimum NVQ/Diploma Level 2 in Healthcare or equivalent
  • Experience in delivering care in a healthcare or people-centered environment
  • Ability to work independently and as part of a team
