Role Description:

We are seeking a highly skilled and experienced Care Co-ordinator to join our team in Southwark Council's SEND service. The successful candidate will manage a caseload of children and young people with EHCPs, working closely with families, schools, and partner agencies to ensure coordinated and effective support.

Main Responsibilities:

Key responsibilities include:

  • Managing school placements and supporting families through the process.
  • Participating in annual reviews to track the progress of children and young people with SEND.
  • Ensuring that children and young people with SEND receive the support they need to succeed.
Requirements:

To succeed in this role, you will need to be highly organised with strong case management skills, capable of handling complex cases and navigating the EHCP process with confidence. Key requirements include:

  • A good understanding of SEND legislation and the EHCP process.
  • The ability to produce high-quality reports and ensure compliance with statutory requirements.
  • Commitment to ongoing professional development and continuous improvement.
  • A full Enhanced DBS check is required for this post.
Salary and Benefits:

We offer a competitive salary of £41,580 - £47,628 per annum (full-time equivalent), plus a range of benefits including professional development opportunities and the chance to make a lasting impact on the lives of young people.
