Compassionate Home Care Professional

About the Role:- Provide person-centred care in line with best practices, policies, and regulatory requirements, under the guidance of the Care Manager and Deputy Manager.
- Support Service Users to maintain skills, personal interests, and social connections with the local community.
- Follow individual care plans and report any changes in service user needs to management.
- Work collaboratively with the care team, families, and multi-disciplinary professionals to ensure the best outcomes for Service Users.
- Maintain accurate records of care provided and promptly report any concerns regarding Service Users' health or wellbeing.
- A valid driving licence, own vehicle, and clean driving record are essential as travel between client homes is required.
- Previous experience in a care role is desirable but not essential. A commitment to learning and working towards a relevant qualification (RQF in Social Care) is encouraged.
- Strong communication skills (both verbal and written), a caring nature, and a good understanding of person-centred care principles.
- Reliability, empathy, professionalism, and a genuine desire to improve the lives of others.
- Ongoing training and professional development opportunities.
- Flexible working hours on a rota basis.
- If you are willing / looking to relocate to Jersey, support will be provided to help you settle in.
The estimated salary for this role is £24,000-£34,000 per annum, depending on experience.
