Part Time Community Frailty Rehabilitation Specialist Physiotherapist

About the Role:

We are seeking a highly skilled and experienced Part Time Community Frailty Rehabilitation Specialist Physiotherapist to join our multidisciplinary team in Plymouth, the South Hams, and West Devon. The successful candidate will play a critical role in our Community Frailty Virtual Ward project, working closely with consultants, pharmacists, dietitians, and other healthcare professionals to provide seamless, integrated care.

Key Responsibilities:
  1. Provide specialist physiotherapy assessments and interventions for individuals admitted to the Frailty Virtual Ward.
  2. Work with the wider multidisciplinary team to provide high-quality, patient-centred care.
  3. Supervise and educate Physiotherapy students, as well as other healthcare professionals.
Requirements:
  • Degree in Physiotherapy and registration with the Health and Care Professions Council (HCPC).
  • Substantial relevant experience within the NHS, including experience as a junior in a broad rotational post providing a broad range of experience in orthopaedics, elderly care, medical, surgical, neurological, and musculoskeletal conditions.
  • Understanding of PT outcome measures and ability to work autonomously and set own priorities.
  • Ability to analyse professional and ethical issues, supervise, and appraise staff.
